The purpose of telehealth is to provide patients with the opportunity to consult, receive diagnosis and treatment for a medical condition using telecommunication tools such as interactive audio, video or other 2-way forms of communication accepted by the recommendation for each state.

I am consenting to allow 24hr Urgent Doc and their staff of doctors, nurse practitioners, certified medical assistants, medical assistants, and customer service team to provide care using telehealth services.

I agree to be honest and transparent with regards to the medical information, symptoms, medications taking, current and past medical conditions so that the providers at 24hr Urgent Doc can best service me based on the request submitted.

 I acknowledge that the services provided by 24hr Urgent Doc is not a guaranteed cure, and if an in-person evaluation or referral is determined to be a better solution, 24hr Urgent Doc will notify me and refund my service fee if no prescription was issued.

I understand that it is my responsibility to alert my physician if I experience a negative reaction to any treatment provided by 24hr Urgent Doc, if necessary, call 911, or seek treatment at the nearest emergency room.

By using this website or completing a consult with 24hr Urgent Doc (the “Service”) you agree to be bound by these Terms of Use (this “Agreement”), whether or not you register or simply submit as a guest.

 By using the Websites or the Service, you give 24hr Urgent Doc permission to create store and/or deliver to you, personal health data, medical records and other information.

24hr Urgent Doc is not responsible for any incorrect or inaccurate content posted on the Websites or in connection with the Service, whether caused by users of the Websites, Members or by any of the equipment or programming associated with or utilized in the Service. 24hr Urgent Doc is not responsible for the conduct, whether online or offline, of any user of the Websites or Member of the Service. 24hr Urgent Doc assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, user or Member communications.

I hereby acknowledge that 24hr Urgent Doc and its affiliates are released from any liability which may occur through the unauthorized use of audio, video, or other electronic transmission.

I agree that the initial fee only covers the initial evaluation, and I am responsible for any and all additional cost of medication at the pharmacy or recommended labs if needed.

I consent to what I have read, or have had the consent read to me, and I agree to all the information herein.
